Description

Ambulatory Care Sensitive Conditions (ACSC) are conditions resulting in a hospital visit (to any department) where the underlying reason (health condition) could have been treated in the community rather than requiring treatment at a hospital (i.e., hospitalisation is potentially avoidable or preventable). The codelist here includes the subset - chronic conditions.
